Job Title: Discharge Coordinator

Job Summary:

Livewell Southwest is seeking a highly skilled and compassionate Discharge Coordinator to join our team. As a key member of our Mental Health and Learning Disabilities Services, you will play a vital role in supporting individuals with complex health and social needs to achieve their recovery goals.

Key Responsibilities:
  • Develop and implement discharge plans that meet the unique needs of each individual, working closely with multidisciplinary teams and community agencies.
  • Identify and address social, housing, and financial barriers to discharge, ensuring timely and effective resolution.
  • Collaborate with patients, families, and carers to empower them to make informed decisions about their care and support.
  • Monitor progress, identify areas for improvement, and implement changes to optimize discharge outcomes.
  • Provide education and support to patients and families on discharge planning, mental health, and well-being.
  • Develop and maintain effective relationships with community agencies, healthcare providers, and other stakeholders to ensure seamless care transitions.
  • Contribute to the development and implementation of policies, procedures, and best practices related to discharge planning and coordination.
Requirements:
  • NVQ Level 5 or equivalent qualification in a relevant field.
  • Significant experience working with individuals with mental health diagnoses, preferably in an administrative or coordination role.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team, with a strong focus on patient-centered care.
  • Knowledge of local health and social care systems, including community resources and services.
  • Understanding of the Mental Health Act and safeguarding processes.
