Discharge Coordinator

We are seeking a highly skilled and experienced Discharge Coordinator to join our team at Livewell Southwest. As a Discharge Coordinator, you will play a vital role in ensuring the smooth transition of patients from our in-patient services to their communities.
Key Responsibilities:- Identify and address social, housing, and financial issues that may impact a patient's discharge.
- Develop and implement discharge plans that meet the individual needs of each patient.
- Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and community agencies, to ensure a seamless discharge process.
- Monitor patient progress and adjust discharge plans as needed to ensure timely and effective discharge.
- Relevant experience working with individuals with mental health needs.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
