Job Summary

Livewell Southwest is seeking a skilled Discharge Coordinator to join our team. As a Discharge Coordinator, you will play a vital role in supporting individuals with complex health and social needs, promoting dignity, respect, and independence.

Key Responsibilities

  • Develop a therapeutic rapport with patients and involve them in the discharge planning process.
  • Proactively identify barriers to discharge and liaise with relevant parties to prevent delays.
  • Monitor the progress of patients and be mindful of bed availability.
  • Work closely with a number of agencies and disciplines to meet the needs of patients.
