Job Summary

Livewell Southwest is seeking a highly motivated and empathetic individual to join our team as a Discharge Support Worker - Mental Health Services. As a key member of our multidisciplinary team, you will play a vital role in supporting individuals with mental health needs to achieve their recovery goals and successfully transition back into their community.

Key Responsibilities

  • Assist the multidisciplinary team in delivering high-quality care to individuals with mental health needs, focusing on their direct needs and promoting their recovery and independence.
  • Provide practical and emotional support to individuals, their relatives, and carers, helping them to access community resources and services.
  • Work collaboratively with other professionals, including nursing staff, doctors, and social workers, to ensure a comprehensive and coordinated approach to care.
  • Develop and maintain relationships with individuals, their families, and carers, promoting dignity, respect, and independence.
  • Contribute to the assessment, planning, implementation, and evaluation of care, ensuring that individuals receive the support they need to achieve their goals.
  • Record and maintain accurate and up-to-date records of individual care plans, using the Care Programme Approach (CPA) process.
  • Participate in regular team meetings and contribute to the development of care plans and strategies.
  • Work in accordance with Livewell Southwest's policies and procedures, ensuring that individuals receive high-quality care and support.

Requirements

  • Relevant experience of working with individuals with mental health needs, preferably in a multidisciplinary team setting.
  • Excellent communication and interpersonal skills, with the ability to build relationships with individuals, families, and carers.
  • Ability to work collaboratively as part of a team, contributing to the development of care plans and strategies.
  • Knowledge of the Care Programme Approach (CPA) and ability to record and maintain accurate and up-to-date records.
  • Ability to work in a fast-paced environment, prioritizing tasks and managing competing demands.
  • Commitment to promoting dignity, respect, and independence for individuals with mental health needs.
