Patient Pathway Coordinator

Job Summary

The post holder will be the point of contact for all administrative issues relating to patients' pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support to their wider team in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.

Main Responsibilities
  • Process and log all referrals, including paper and electronic referrals, in line with Trust policy and specialty standard operating procedures.
  • Manage 18-week referral to treatment (RTT) pathway tracking to ensure all outpatient and elective patient journeys are coordinated, including booking appointments and theater sessions, and ensuring clinic outcomes are recorded accurately.
  • Utilize Trust IT systems and specialty databases to ensure relevant and accurate information is recorded.
  • Demonstrate high levels of customer care and serve as an ambassador for customer care within the Trust.

Job Requirements
  • Computer literacy
  • Good attention to detail and ability to initiate and coordinate change
  • Essential: GCSE or equivalent
  • Desirable: Previous NHS experience and knowledge of healthcare administrative systems and processes
