Family Practitioner Role

We are seeking a compassionate and dedicated Family Practitioner to join our Women and Families team at The Nelson Trust.

Key Responsibilities
  • Deliver trauma-informed, holistic services to families facing substance misuse challenges.
  • Offer practical and emotional support, facilitate therapeutic interventions, and collaborate with internal teams and external agencies to help families achieve their goals.
  • Manage a caseload of families, providing tailored support to meet their needs.
  • Facilitate individual and group therapy sessions, develop action plans, and support families in accessing specialist services.
  • Conduct assessments, participate in multi-agency meetings, and contribute to social care, safeguarding, and child protection efforts.
Requirements
  • Experience in working with families affected by substance misuse.
  • Knowledge of trauma-informed practice and holistic support.
  • Ability to work collaboratively with internal teams and external agencies.
  • Strong communication and interpersonal skills.
  • Ability to maintain accurate records and participate in ongoing professional development.
