About the Role

We are seeking a highly skilled and motivated Registered Mental Health Nurse to join our North East Kent Home Treatment Team.

As a key member of our multidisciplinary team, you will be responsible for assessing, planning, delivering, and evaluating care for patients with memory impairment and/or functional mental ill health.

You will work collaboratively with other disciplines, patients, and their carers to develop holistic care plans that address the needs of each individual.

Key Responsibilities
  1. Manage a defined caseload of patients with functional and organic mental illness under supervision.
  2. Conduct comprehensive assessments that take into account mental, physical, social, cultural, psychological, spiritual, and environmental factors.
  3. Work collaboratively with other disciplines to develop and implement care plans that address the needs of each patient.
  4. Deliver and evaluate safe, competent, person-centred care in partnership with patients and their carers.
