About the Role

We are seeking a highly skilled and motivated Registered Mental Health Nurse to join our North East Kent Home Treatment Team. As a key member of the multidisciplinary team, you will be responsible for assessing, planning, delivering, and evaluating care for patients with memory impairment and/or functional mental ill health.

The ideal candidate will have experience of acute mental health care, nursing a range of mental health conditions, and knowledge of current best practice in providing holistic interventions. You will also have the ability to travel across a large geographical area and possess a robust understanding of risk assessment and risk management.

Key Responsibilities
  1. To manage a defined caseload of patients with functional and organic mental illness under supervision.
  2. To carry out comprehensive holistic assessments, taking account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors.
  3. To work collaboratively with other disciplines, patients, and their carers to agree a holistic person-centred care plan.
  4. To deliver and evaluate safe, competent, person-centred care in partnership with the service user and their carers to support recovery.
