About the Role

We are seeking a highly skilled and motivated Registered Mental Health Nurse to join our North East Kent Home Treatment Team. As a key member of the multidisciplinary team, you will be responsible for assessing, planning, delivering, and evaluating care for patients with memory impairment and/or functional mental ill health.

The successful candidate will have a strong clinical background in acute mental health care, with experience of nursing a range of mental health conditions. You will be able to travel across a large geographical area and have a robust understanding of relevant legislative frameworks and policies relating to care.

Main Responsibilities
  1. To manage a defined caseload of patients with functional and organic mental illness under supervision.
  2. To carry out comprehensive holistic assessments, taking account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors.
  3. To work collaboratively with other disciplines, patients, and their carers to agree a holistic person-centred care plan.
  4. To deliver and evaluate safe, competent, person-centred care in partnership with the service user and their carers to support recovery.
