About the Role

We are seeking a highly skilled and motivated Registered Mental Health Nurse to join our North East Kent Home Treatment Team.

The successful candidate will have a strong clinical background in mental health and experience in working with patients with memory impairment and functional mental ill health.

Key Responsibilities
  1. Manage a defined caseload of patients with functional and organic mental illness, providing comprehensive assessments and developing holistic care plans.
  2. Work collaboratively with multidisciplinary teams, patients, and carers to deliver safe and competent person-centred care.
  3. Conduct regular reviews and evaluations to ensure the effectiveness of care plans and make adjustments as needed.
  4. Participate in ongoing professional development to stay up-to-date with best practices and evidence-based treatments.
