Our community comprises individuals of various ages, including the elderly, those recently released from medical facilities, individuals facing terminal illnesses, and those with physical challenges.

Our nursing teams collaborate closely with local Primary Care Networks to ensure that patient care is comprehensive and interconnected. We take the time to understand our patients, believing in the importance of treating the whole person.

Key Responsibilities

In your role as a Community Nursing Partner, you will:

  • Collaborate with a team to deliver care to patients and their families within the community.
  • Assess and monitor the quality of care provided to patients.
  • Provide constructive feedback to management regarding care standards.

Your responsibilities will include implementing care plans tailored to support individuals living at home, in care facilities, or other environments, ensuring a positive experience for those utilizing our services.
