We are seeking a highly skilled and compassionate Nursing Associate to join our community care team at Kent Community Health NHS Foundation Trust. As a Nursing Associate, you will play a vital role in delivering high-quality patient care and supporting our patients to live independently in their own homes.
Key Responsibilities- Provide person-centered care to patients with complex needs, working closely with our multidisciplinary team to develop and implement individualized care plans.
- Conduct regular assessments and reviews to ensure patients receive the best possible care and support.
- Work collaboratively with healthcare professionals to ensure seamless care transitions and effective communication.
- Develop and maintain strong relationships with patients, families, and carers to ensure their needs are met and their voices are heard.
- Contribute to the development and implementation of new care initiatives and services to improve patient outcomes and experiences.
- Registered with the NMC as a Nursing Associate registrant.
- Knowledge of nursing theory and best practice at Nursing Associate level.
- Experience of working as part of a team and providing care to patients with complex needs.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to prioritize care delivery and manage competing demands in a fast-paced environment.
