We are seeking a highly skilled and experienced Community Nurse Leader to join our team at Kent Community Health NHS Foundation Trust. As a Band 7 PCN Lead, you will play a key role in leading and managing our community nursing teams, working closely with operational managers and other stakeholders to deliver high-quality patient care.
Key Responsibilities- Lead and manage community nursing teams to ensure delivery of high-quality patient care
- Develop and implement policies and procedures to improve patient outcomes and service delivery
- Work closely with operational managers and other stakeholders to ensure effective communication and collaboration
- Provide coaching and mentoring to team members to support their professional development
- Contribute to the development of service plans and strategies to improve patient care and service delivery
- Registered health care professional (NMC registration)
- Health related degree and evidence of relevant postgraduate development at Level 6
- Evidence of postgraduate specialist qualifications relevant to area of practice
- Management or supervisory qualification/programme of study
- Preparing to Teach in the Lifelong Learning Sector/Diploma in Teaching in the Lifelong Learning Sector, Postgraduate Certificate of Education or Certificate in Education
- Experience with the use of Microsoft Office
