Team Leader

We are seeking a highly skilled and experienced Team Leader to join our Community Nursing Team in the Maidstone area. As a key member of our team, you will play a vital role in delivering high-quality patient care and leading a team of nurses to achieve excellent outcomes.
Key Responsibilities- Lead and manage a team of nurses to provide case load management for housebound patients, supporting their nursing needs and enabling them to self-care.
- Develop and implement advanced skills to manage the complexity of patients seen, working closely with the community nursing service.
- Act as a lead and senior practitioner to support junior staff in assessment and reassessment of service users and/or pathways with complex needs.
- Work effectively with a variety of people at all organisational levels across different sectors, including areas of social deprivation.
- Communicate complex, sensitive information to individuals and groups, using strong influencing and negotiating skills.
- Valid UK NMC Pin and evidence of Level 6/7 professional development.
- Minimum 5 years experience in area of practice, with experience as a mentor.
- Understanding of National Service Frameworks and NICE guidance relevant to care setting and its application across healthcare services.
- Ability to manage time effectively, prioritize and work under pressure to meet deadlines.
- Valid driving license and access to a car for work purposes.
