The Kent Community Health NHS Foundation Trust is seeking a qualified and skilled Team Leader/Specialist Practitioner District Nurse to join our Community Nursing Team in the Maidstone area.
Main DutiesAs a Band 6 Team Leader, you will work closely with patients, families, friends, and carers to provide high-quality care and support. You will also act as a lead and senior practitioner to support junior staff in assessing and reassessing service users with complex needs.
You will be responsible for managing a caseload of patients and providing guidance and support to junior staff. You will also be involved in developing and implementing care plans, and ensuring that patients receive the best possible care.

Person SpecificationWe are looking for a qualified and experienced Team Leader/Specialist Practitioner District Nurse who has a valid UK NMC Pin and evidence of Level 6/7 professional development.
You will need to have a minimum of 5 years' experience in the area of practice and be a mentor. You will also need to have experience of leading and managing teams, as well as IT experience.
You will need to have a strong understanding of National Service Frameworks and NICE guidance relevant to the care setting, as well as evidence of continued relevant professional development and implementation in practice within the last 12 months.
You will also need to have strong influencing and negotiating skills, as well as the ability to communicate complex and sensitive information to individuals and groups.
