About the Role

We are seeking a highly skilled and experienced Team Leader to join our Community Nursing Team in the Maidstone area. As a Team Leader, you will play a key role in leading and managing a team of nurses to provide high-quality care to patients in their homes.

Key Responsibilities
  • Lead and manage a team of nurses to provide case load management for housebound patients.
  • Develop and implement care plans to meet the needs of patients with complex needs.
  • Work closely with other healthcare professionals to ensure seamless care delivery.
  • Manage and lead change to improve patient outcomes and team performance.
  • Provide coaching and mentoring to junior staff to develop their skills and knowledge.
Requirements
  • Valid UK NMC Pin and evidence of L6/L7 professional development.
  • Minimum 5 years experience in area of practice, including experience as a mentor.
  • Experience of leading/managing teams and IT experience.
  • Understanding of National Service Frameworks and NICE guidance relevant to care setting.
  • Ability to communicate complex, sensitive information to individuals and groups.
  • Strong influencing and negotiating skills.
