About the Role

We are seeking a highly skilled and experienced Team Leader/Specialist Practitioner District Nurse to join our Community Nursing Team in the Maidstone area.

The successful candidate will work closely with patients, families, friends, and carers to provide high-quality care and support. As a Band 6 Team Leader, you will be responsible for leading and managing a team of nurses, providing guidance and support to junior staff, and contributing to the development of advanced skills and knowledge within the team.

The community nursing service provides case load management for housebound patients, enabling them to self-care and manage their nursing needs. Nurses within the team are supported to develop advanced skills to manage the complexity of patients seen, and are also supported through the Specialist Practitioner course, recognizing community nursing as a specialism within the NMC.

Main Responsibilities

  • Lead and manage a team of nurses, providing guidance and support to junior staff
  • Contribute to the development of advanced skills and knowledge within the team
  • Provide high-quality care and support to patients, families, friends, and carers
  • Manage and prioritize care delivery in a professional manner, considering Trust policies and procedures
