We are seeking a highly skilled and experienced Team Leader to join our Community Nursing Team in the Maidstone area. As a key member of our team, you will play a vital role in delivering high-quality patient care and leading a team of nurses to achieve excellent outcomes.
Key Responsibilities- Lead and manage a team of nurses to provide case load management for housebound patients, supporting their nursing needs and enabling them to self-care.
- Develop and implement effective care plans, working closely with patients, families, and carers to achieve optimal health outcomes.
- Act as a senior practitioner, supporting junior staff in assessment and reassessment of service users with complex needs.
- Collaborate with other healthcare professionals to ensure seamless care delivery and achieve excellent patient outcomes.
- Contribute to the development of the community nursing service, identifying areas for improvement and implementing changes to enhance patient care.
- Valid UK NMC Pin and evidence of Level 6/7 professional development.
- Minimum 5 years' experience in community nursing, with a proven track record of leadership and mentoring.
- Experience of leading/managing teams, with a strong understanding of National Service Frameworks and NICE guidance.
- Excellent communication and interpersonal skills, with the ability to work effectively with a variety of people at all organisational levels.
- Strong influencing and negotiating skills, with the ability to manage time effectively and prioritize tasks under pressure.
