Position Overview

This role presents a unique opportunity to become a vital member of the Paediatric Liaison Service, which is an integral part of the Nottinghamshire Healthy Families Team Programme.

The successful candidate will play a key role in enhancing the Nottinghamshire Countywide Paediatric Liaison Service, collaborating closely with the existing team of Children and Young People's Liaison Practitioners. The primary focus will be on facilitating communication with Bassetlaw Hospital, while also demonstrating flexibility to engage with Sherwood Forest Hospitals and Nottingham University Hospitals as necessary to fulfill service requirements.

The individual in this position will be tasked with ensuring effective communication between acute healthcare settings, the Healthy Families Team, and primary care providers. They will work diligently to identify children and young people within the County who may be vulnerable, have heightened needs, safeguarding issues, or require ongoing support following visits to Accident & Emergency or admissions to children's wards/Neonatal Units. Timely sharing of information among staff working with at-risk children in acute environments and Healthy Families Teams will be a critical responsibility.

Additionally, the role includes overseeing the coordination of the Care of the Next Infant (CONI) programme.

A significant aspect of this position is the need to adapt and respond to the evolving needs of the service.

We promote flexible working arrangements across acute settings, community environments, and home visits.

Key Responsibilities

1. Facilitate communication and information exchange between acute healthcare facilities and community services.

2. Provide early guidance and support to parents of infants admitted to neonatal units and assist in discharge planning from acute care.

3. Coordinate the Care of the Next Infant Scheme (CONI).

4. Ensure timely communication of critical information to Healthy Families Teams regarding serious incidents, safeguarding concerns, and Sudden Infant Death.

5. Collaborate with safeguarding teams in both acute and community settings to identify trends and issues related to safeguarding.

6. Work with acute settings to assess risks and share pertinent information with Healthy Families Teams.

7. Foster effective teamwork by sharing knowledge and encouraging a supportive environment that promotes best practices and positive outcomes.

8. Maintain visibility and accessibility to acute staff, service users, and families for guidance and health information.

9. Engage with the Nottinghamshire Safeguarding Board and adhere to organizational safeguarding policies, proactively identifying concerns and reporting them to senior team members.

10. Participate in safeguarding supervision in accordance with organizational policies.

11. Conduct audits to ensure safe and evidence-based practices are upheld.

12. Uphold confidentiality and protect client information in compliance with the Data Protection Act and Information Governance Policy.

13. Maintain accurate and up-to-date records in line with organizational policies and NMC guidelines, including data entry in SystmOne.

14. Access clinical supervision as per organizational policy.

15. Commit to continuous personal development and maintain competencies required for the role.

16. Contribute to mentorship and preceptorship to enhance staff development and performance quality.

17. Engage in research, service development, and innovation initiatives.

18. Adhere to NMC, national, and organizational policies, including health and safety regulations.

Qualifications

Essential

  • 1st level Registration with current NMC registration
  • Registered Specialist Practitioner (Health Visitor / School Nurse)

Desirable

  • Recognized teaching qualification (ENB 998/730/FLIPS or equivalent).
  • Evidence of relevant further professional development.
