About the Role

We are seeking a highly skilled and experienced Children and Young Peoples Liaison Specialist to join our team at Nottinghamshire Healthcare NHS Foundation Trust.

The successful candidate will be responsible for facilitating effective communication and information sharing between acute settings, Healthy Families Teams, and Primary Care.

Key Responsibilities:

  • Facilitate communication and information sharing between acute settings, Healthy Families Teams, and Primary Care.
  • Share relevant information with Healthy Families Teams to proactively support discharge from acute settings.
  • Coordinate the Care of the Next Infant Scheme (CONI).
  • Share information in a timely way to Healthy Families Teams relating to serious incidents, Safeguarding concerns, and Sudden Infant Death.
  • Work with acute and community Safeguarding teams to highlight trends, issues, and incidents related to Safeguarding.
  • Identify risk and share relevant information with Healthy Families Teams.
  • Promote effective team working, sharing knowledge and information, and encouraging a supportive environment to promote good working practice and outcomes.
  • Be visible and accessible to acute staff, service users, and families for signposting and health advice.
  • Signpost and provide information to parents and young people on the Healthy Families Teams, communicating information in an understandable form.
  • Work with Nottinghamshire Safeguarding Board and organisational Safeguarding child and adult policies and procedures.
  • Access and participate in Safeguarding supervision as per organisational policy.
  • Complete audit activities to ensure appropriate, safe, and evidence-based practice.
  • Act in accordance with the Data Protection Act and Information Governance Policy to ensure confidentiality and protection of client information.
  • Maintain accurate, up-to-date contemporaneous records in line with the organisation policy and NMC guidance.
  • Access Clinical Supervision as per organisational policy.
  • Demonstrate continuous personal development and maintenance of competencies required for the role.
  • Undertake mentorship, preceptorship as appropriate to contribute staff development and quality of performance.
  • Initiate and/or contribute to research, service development, and innovation.
  • Work within NMC, National, and organisational policies, including Health and Safety.
  • Promote equality, diversity, and rights, treating others with dignity and respect at all times.
  • Undertake any other duties which may reasonably be required within the remit of the role.

Requirements

  • 1st level Registration with current NMC registration.
  • Registered Specialist Practitioner (Health Visitor / School Nurse).
  • Recognised teaching qualification (ENB 998/730/ FLIPS or equivalent).
  • Evidence of relevant, further professional development.
  • Commitment to Equality, Diversity, and Inclusion (EDI) and how it applies to the role.
  • Experience within Public Health Nursing Practice.
  • Experience of working in multi-disciplinary teams.
  • Demonstrate knowledge and understanding of current children & family health agenda.
  • Have knowledge of research and audit.
  • Proven knowledge & skills in Safeguarding children, young people & adults.
  • Proven communications skills - written, oral & IT.
  • Excellent interpersonal skills (e.g. listening, negotiation, self-awareness, respectful challenge, problem-solving).
  • Proven organisation skills (e.g. time management, prioritisation, self-management, and motivation).
  • Ability to work independently and as part of a team.
  • Ability to support change and transformation across service.
  • Flexibility.
  • Full driving license and the ability to travel between locations.
