Position Overview
We are seeking a dedicated Social Worker to join our Community Stroke and Neuro Rehabilitation Team (CSNRT). This role involves providing essential social work assistance, conducting strength-based evaluations, and developing personalized support plans in accordance with the Care Act 2014 for adults undergoing stroke rehabilitation.
Key Responsibilities
- Conduct strength-based assessments and deliver tailored support to adults with care needs.
- Promote independence and empower individuals to recognize their potential for change.
- Collaborate effectively with a diverse range of adults while applying a comprehensive understanding of the Adult Social Care legislative framework.
- Respond effectively to safeguarding inquiries in alignment with the Care Act 2014 and local policies.
- Work both autonomously and as part of a multidisciplinary team, demonstrating familiarity with the Mental Capacity Act 2005.
Candidate Requirements
The ideal candidate will possess a relevant social work qualification and be registered with Social Work England. Strong communication skills and the ability to work proactively with individuals are essential. You should be committed to delivering positive outcomes for residents and capable of professionally challenging practices to ensure the best results.
