Key Responsibilities

  • Establish Strong Collaborations: Collaborate closely with the voluntary sector to develop and maintain partnerships that benefit our patients and the surrounding community.
  • Address Health Disparities: Design and execute initiatives aimed at mitigating health inequalities, ensuring equitable access to necessary care for all.
  • Assist Patients and Community Members: Serve as a primary contact, offering vital support to patients and their families, while engaging with local residents to identify and address their health needs.

Job Responsibilities

The successful candidate will collaborate with colleagues to identify areas of demand on hospital services that can be alleviated through engagement with volunteers, the voluntary sector, local residents, and stakeholder groups, fostering stronger, more resilient communities that reduce pressure on the health and care system. The work of the Community Outreach Coordinators will align with the NHS England CORE20PLUS5 framework for addressing health inequalities.

This role supports the development of the Patient Experience and Involvement strategy, ensuring that patients and local residents are central to all our initiatives.

Qualifications and Professional Training

Essential

  • Degree-level education or equivalent experience in community engagement or health/social care services.

Experience and Knowledge

Essential

  • Prior experience in a community or public/patient engagement role.
  • Understanding of health disparities and their impact on health outcomes, along with familiarity with the NHS England CORE20PLUS5 approach.
  • Experience in planning and executing public involvement activities, integrating feedback into work programs and campaigns.
  • Knowledge of media engagement and its role in community outreach.

Desirable

  • Experience within the NHS or local voluntary community organizations, with an understanding of NHS structures and challenges.
