Job Summary:

NHS Scotland is seeking a skilled and dedicated Community Health Nurse to join our team. As a Community Health Nurse, you will provide high-quality, patient-centered care to individuals aged 5 years and over in a community setting.

Key Responsibilities:

  • Provide skilled clinical interventions, including health education and health promotion, to promote independent living for patients.
  • Work in partnership with other members of the primary healthcare team and service users to deliver effective care.
  • Manage a high turnover of patients with nursing interventions, prioritizing care needs to respond to both planned and unplanned care.
  • Collaborate with GP practices, other primary care teams, and secondary care colleagues to ensure seamless care delivery.

Requirements:

  • Live PIN with the NMC UK.
  • Ability to work in a fast-paced environment with a high turnover of patients.
  • Strong communication and interpersonal skills to work effectively with patients, families, and healthcare teams.
  • Knowledge of community nursing practices and standards.

Working Arrangements:

This is a full-time position, working 37 hours per week. You may be required to work up to an additional 30 minutes per week for a temporary period to accommodate the new reduced working week.
