We are seeking a dedicated and compassionate Registered Nurse to join our Community Palliative Care team based out of our client's Hospice in Hertfordshire.
About the RoleThis is a full-time position, but we are open to discussing part-time hours opportunities. As this role involves delivering services in the community, it is essential that you have a car and a valid driver's license.
Key Responsibilities- Deliver high-quality Palliative and End of Life nursing care as part of the Community MDT, coordinating assessment, planning, and evaluation of individualized patient-centered care.
- Establish effective working relationships with multiprofessional community teams.
- Provide advice and support to patients and their families affected by life-limiting disease at the patient's home or other care settings.
- Act as the patient or carer advocate by liaising with other colleagues, including the GP and hospital teams.
