Job Title: Palliative Care Nurse

We are seeking a dedicated and compassionate Registered Nurse to join our Community Palliative Care team based out of our client's Hospice in Berkhamsted, Hertfordshire.

This is a developmental role, ideal for a Registered Nurse looking to transition into Palliative Care. You will have the opportunity to progress to a Band 6 post within 6-12 months, upon completion of a competency structured programme.

The team works 7 days a week from 9:00 - 5:00, and as this role delivers services in the community, it is essential that you are a car driver and have access to a vehicle.

Key Responsibilities:
  1. Deliver high-quality Palliative and End of Life nursing care as part of the Community MDT, coordinating assessment, planning, and evaluation of individualised patient-centred care.
  2. Establish effective working relationships with multiprofessional community teams, providing advice and support to patients and their families affected by life-limiting disease.
  3. Refer to appropriate services to ensure patients have the support they require, acting as the patient or carer advocate by liaising with other colleagues, including the GP and hospital teams.

Our client is a UK Charity with a purpose-built Hospice based in Berkhamsted, Hertfordshire, dedicated to providing patient-centred palliative care for adults with serious illnesses.

The Community Palliative Care team delivers individualised care for patients, their families, and carers, supporting their wishes and preferences at home.

Person Requirements:
  1. Registered Nurse with full UK statutory NMC registration.
  2. A strong desire to move your career into Palliative Care.
  3. Proficient in the use of IT systems.
  4. Good organisational skills.
  5. Excellent written and verbal communication skills.
  6. Ability to work effectively in an emotionally demanding environment.
  7. Flexible working to defined shift patterns, including weekend working.
  8. Car owner and driver.
