Job Title: Community Palliative Care Nurse

We are seeking a dedicated and experienced Senior Staff Nurse or Paramedic to join our Community Palliative Care team based out of our client's Hospice in Berkhamsted, Hertfordshire.

About the Role:

This is a full-time position, but we are open to hearing from candidates seeking to negotiate a part-time hours opportunity. The team works 7 days a week from 9:00 to 5:00. As this role delivers services in the community, it is essential that you are a car driver and have access to a vehicle.

Key Responsibilities:
  • Deliver high-quality Palliative and End of Life nursing care as part of the Community MDT, coordinating assessment, planning, and evaluation of individualized patient-centered care.
  • Provide advice, psychological support, and symptom control for patients with an associated life-limiting condition and support their families.
  • Work autonomously and as part of a team providing care and support to patients in their home, in outpatient clinics, and Wellbeing groups.
  • Make referrals to appropriate services to ensure patients have the support they require.
  • Participate in evaluation, clinical audit, team meetings, awaydays, supervision, and continuous professional development.

Requirements:
  • Registered Nurse or Paramedic with full UK statutory registration (NMC/HCPC) as applicable.
  • Current or recent experience in palliative care.
  • Nurse prescribing qualification or willingness to complete.
  • Evidence of Continuing Professional Development.
  • Proven experience of using IT systems.
  • Good organizational skills.
  • Excellent written and verbal communication skills.
  • Ability to work effectively in an emotionally demanding environment.
  • Flexible working to defined shift patterns, including weekend working.
  • Car owner and driver.
