About the Role

We are seeking an enthusiastic Registered Nurse to join our Community Palliative Care team. As a key member of our team, you will provide high-quality Palliative and End of Life nursing care as part of the Community MDT, coordinating assessment, planning, and evaluation of individualised patient-centred care.

As a Palliative Care Nurse, you will establish effective working relationships with multiprofessional community teams, provide advice and support to patients and their families affected by life-limiting disease, and act as the patient or carer advocate by liaising with other colleagues, including the GP and hospital teams.

Key Responsibilities
  • Deliver high-quality Palliative and End of Life nursing care
  • Coordinating assessment, planning, and evaluation of individualised patient-centred care
  • Establish effective working relationships with multiprofessional community teams
  • Provide advice and support to patients and their families
  • Act as the patient or carer advocate
