Nurse Care Coordinator

We are seeking a skilled Registered Nurse to join our team as a Clinical Team Lead. As a key member of our care delivery team, you will be responsible for driving and running the unit alongside clinical duties in the home. This includes observation, medication, reviewing and evaluating individual care plans, and providing clinical guidance to the care team.
Key Responsibilities:
- Lead the care team and ensure high-quality patient care is delivered in line with our clinical standards.
- Develop and implement individualised care plans for residents, working closely with the multidisciplinary team.
- Monitor work areas and practices to ensure they are safe and conform to relevant standards, policies, and legislation.
- Provide coaching and guidance to colleagues, promoting clinical excellence and best practice.
