Nurse Care Coordinator

We are seeking a dedicated Registered General Nurse to join our Hospice of St Francis team in delivering exceptional palliative care.
Job DescriptionThis role involves assessing, planning, implementing, and evaluating care for patients and their families, ensuring continuity through comprehensive recording and handovers. As a member of the multi-professional team, you will maintain high standards of care, participate in guideline development, and supervise Health Care Assistants.
Key Responsibilities- Maintaining standards of care within the inpatient unit and ensuring the highest quality care
- Assessing, planning, implementing, and evaluating care for patients and their families
- Ensuring continuity of care through comprehensive recording and handovers
- Participating in guideline development and auditing
- Supervising Health Care Assistants
- Maintaining safe custody and administration of drugs
- Establishing caring relationships and providing support, advice, and referrals
- Providing leadership for the inpatient unit team as needed
- Registered General Nurse (Level 1)
- Evidence of Continuing Professional Development
- Passion for palliative care and desire to learn
- Ability to collaborate with colleagues across different departments
- Knowledge of infection control, health and safety, and vulnerable adults
- Experience of facilitating partnership working, decision making, and conflict resolution
- Excellent communication skills verbally and in writing
