Position Overview

The Emergency Care Department is dedicated to providing immediate health services, therapy, and support to ensure individuals can remain in their home or community settings. This role involves caring for patients experiencing acute health issues that may otherwise necessitate hospitalization, as well as facilitating safe discharges from medical facilities.

Nighttime care encompasses critical support such as end-of-life assistance, catheter management, and other urgent interventions that cannot be postponed until the next day. Our night nursing services are designed to promote recovery and foster independence, prioritizing rehabilitation and enablement through active engagement with patients.

At Kent Community Health NHS Foundation Trust, we collaborate with leading healthcare institutions to develop innovative care solutions, including virtual wards for patients with acute respiratory conditions, allowing them to receive treatment in the comfort of their homes.

Patients who opt for early discharge from hospitals will be monitored remotely through advanced technology, including applications and wearable devices, while receiving ongoing support from healthcare professionals. The specifics of the technology utilized are currently being explored in partnership with patients and staff.

Key Responsibilities

  • Conduct comprehensive assessments, planning, implementation, and evaluation of holistic patient care.
  • Develop integrated care plans in collaboration with patients.
  • Order necessary equipment and facilitate onward referrals.
  • Serve as a lead practitioner, guiding junior staff in conducting assessments and managing complex patient pathways.
  • Collaborate effectively within a multi-disciplinary team to deliver high-quality patient care.
  • Perform a variety of clinical interventions and monitor for any changes in patients' conditions, taking appropriate actions.
  • Manage a diverse caseload of patients with complex needs on a daily basis.
  • Support the Clinical Lead in team management and operational duties.
  • Provide insights and recommendations to senior management to enhance service delivery and meet organizational objectives.

Qualifications and Experience

Essential:

  • Registered healthcare professional with a minimum of two years' experience in a health improvement setting.
  • Valid UK NMC PIN.

Desirable:

  • Specialist qualifications relevant to the practice area.
  • Health-related degree and evidence of continued professional development.
