About the Role

As a Community Support Specialist at REHAB Basel, you will play a vital role in delivering individualized community support to individuals with intellectual, physical, and mental health challenges. Your primary objective will be to empower these individuals to become valued members of their local communities.

Key Responsibilities

  • Develop and implement educational, recreational, and occupational programs within the community, ensuring best practice in line with up-to-date research on Quality of Life improvements.
  • Liaise with families, multidisciplinary teams, and key community-based personnel to facilitate accessible community activities for service users.
  • Document information, update records, and complete necessary paperwork for communication and accountability purposes.
  • Ensure all interactions with service users are approached with dignity, respect, equality, and incorporate choice.

Requirements

  • Minimum QQI Level 5 in Health and Social Care or equivalent is desirable.
  • Willingness to participate in Rehab Care training program and relevant external training opportunities as they arise.
  • Experience in supporting individuals with intellectual, physical, and mental health challenges.
  • Track record of supporting individuals to identify and experience meaningful experiences in their local community.
