Senior Support Worker

Hazlewood House, a Stonewater housing project, provides a supportive environment for individuals recovering from mental health issues. Our team is dedicated to helping residents achieve their goals and live independently.
As a Senior Support Worker - Team Lead, you will play a key role in delivering person-centered support and leading a team of Support Workers to do the same. Your primary responsibility will be to manage a caseload of residents, supporting and encouraging them to set and achieve their goals.
Key responsibilities include:
- Managing referrals into the service, conducting interviews and assessments, and notifying residents of the outcomes of these activities.
- Inducting new residents into the service and delivering quality support sessions in one-to-one and group settings.
- Developing and implementing individualized support plans to help residents achieve their goals.
- Collaborating with internal and external stakeholders to ensure residents receive comprehensive support.
- Providing guidance and support to junior team members to help them develop their skills and knowledge.
The ideal candidate will possess:
- A Level 3 qualification in Health and Social Care (or equivalent).
- Proven experience of working with vulnerable individuals with support needs.
- Excellent communication and interpersonal skills, with the ability to work effectively with a diverse range of stakeholders.
- Strong leadership and management skills, with the ability to motivate and inspire a team.
- Ability to work under pressure and prioritize tasks effectively.
- Proficiency in Microsoft Office packages and other relevant software.
Appointment to this role will be subject to a satisfactory Disclosure and Barring Service Check, satisfactory references, and possession of a valid Right to Work document.
