About the Role

Hazelmeade House is a leading housing project providing support services for individuals recovering from mental health issues. As our Senior Support Worker, you will play a key role in delivering person-centred support and leading a team of Support Workers to achieve similar outcomes.

Key Responsibilities
  • Manage a caseload of customers, supporting them to achieve their goals and aspirations.
  • Lead and inspire a team of Support Workers to deliver high-quality support services.
  • Develop and implement effective support plans, focusing on customer outcomes and community engagement.
  • Collaborate with internal stakeholders to ensure seamless service delivery and customer satisfaction.
  • Conduct assessments, interviews, and inductions to ensure customers receive the necessary support.
  • Deliver training and development programs to enhance customer skills and knowledge.
  • Monitor and evaluate service performance, identifying areas for improvement and implementing changes as needed.
Requirements
  • Hold a Level 3 qualification in Health & Social Care or equivalent.
  • Proven experience in leading a team and managing caseloads.
  • Excellent communication and interpersonal skills, with the ability to work with diverse stakeholders.
  • Strong knowledge of housing and benefits advice, with the ability to provide guidance and support.
  • Ability to work under pressure and make sound decisions in a fast-paced environment.
  • Proficient in Microsoft Office packages and other relevant software.
