Job Summary:

We are seeking a highly motivated and experienced Senior Support Worker to join our team at Hazelmeade House, a leading housing project for individuals recovering from mental health issues. As a Senior Support Worker, you will play a crucial role in delivering person-centered support and leading a team of Support Workers to achieve outstanding outcomes for our customers.

Key Responsibilities:

  • Manage a caseload of customers, providing support and guidance to help them achieve their goals and aspirations.
  • Lead and inspire a team of Support Workers, promoting a culture of excellence and continuous improvement.
  • Develop and implement effective support plans, ensuring customers receive high-quality services that meet their individual needs.
  • Collaborate with internal and external stakeholders to ensure seamless referrals, assessments, and service delivery.
  • Conduct regular reviews and evaluations to assess customer progress and identify areas for improvement.
  • Provide training and coaching to Support Workers, promoting best practices and industry standards.
  • Contribute to the development of policies and procedures, ensuring compliance with regulatory requirements and Stonewater's values.

Requirements:

  • Level 3 qualification in Health and Social Care (or equivalent).
  • Proven experience in leading a team and managing caseloads.
  • Excellent communication and interpersonal skills, with the ability to work with diverse stakeholders.
  • Strong knowledge of housing and benefits advice, with the ability to provide guidance and support to customers.
  • Ability to work under pressure and prioritize tasks effectively.
  • IT literate and proficient in Microsoft Office packages.
