Locality Coordinator

About the Role
We are seeking a highly skilled and experienced Locality Coordinator to join our team at Torbay and South Devon NHS Foundation Trust. As a key member of our Health and Wellbeing Team, you will play a vital role in coordinating the care and support of our patients, ensuring they receive the best possible outcomes.
Key Responsibilities
- Coordinate the care and support of patients, working closely with health and social care professionals to ensure seamless delivery of services.
- Facilitate regular multi-disciplinary meetings to discuss patient care and ensure effective communication between teams.
- Liaise with health and social care colleagues, voluntary sector representatives, and other stakeholders to promote safe and independent living for complex clients.
- Support the utilisation of case finding tools to identify individuals at risk of loss of independence or unnecessary hospital admission.
- Provide high-quality customer service, recording contact information, prioritising contacts, and providing feedback to referrers and relevant others.
- Maintain IT-based information systems and produce key performance data.
Requirements
- Good general education and a good command of the English language, verbal and written.
- Knowledge of office administration/customer services acquired through training or experience to diploma level or equivalent qualification.
- Knowledge of Health and Social Care administration and processes.
- Experience in using multiple database computer systems and working in an office environment.
- Ability to work well in a team, communicate effectively with a wide range of people, and maintain confidentiality.
