Key Responsibilities

  • Coordinate regular multi-disciplinary meetings to facilitate effective communication and collaboration between health and social care teams.
  • Liaise with health and social care colleagues, voluntary sector representatives, and other stakeholders to ensure seamless care coordination.
  • Support the development and implementation of care plans, ensuring that patients receive the most appropriate and effective care.
  • Provide high-quality customer service, responding to referrals, and ensuring that patients and their families receive timely and effective support.
  • Maintain accurate records and reports, using IT-based information systems to track patient progress and outcomes.

Requirements

  • Good general education and a strong command of the English language.
  • Knowledge of office administration and customer services, preferably within the NHS.
  • Experience of working in a busy and demanding environment, with the ability to prioritize tasks and manage multiple projects.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
