Job Summary

We are seeking a highly skilled and experienced Locality Coordinator to join our team at Torbay and South Devon NHS Foundation Trust. Main Responsibilities
  • Coordinate the care of patients in their own homes, working closely with health and social care professionals to ensure seamless delivery of services.
  • Facilitate regular multi-disciplinary meetings to discuss patient care and ensure effective communication between teams.
  • Develop and maintain relationships with local health and social care providers, voluntary sector organizations, and community groups to ensure a comprehensive range of services are available to patients.
  • Provide high-quality customer service, responding to patient and referrer inquiries, and ensuring timely and effective communication.
  • Manage and maintain accurate records of patient interactions, using IT-based information systems to track progress and identify areas for improvement.

Requirements
  • Good general education and a strong command of the English language, verbal and written.
  • Knowledge of office administration and customer services, acquired through training or experience to diploma level or equivalent qualification.
  • Knowledge of Health and Social Care administration and processes.
  • Experience in using multiple database computer systems and working in an office environment.
  • Ability to work well in a team, communicate effectively with a wide range of people, and maintain confidentiality.
