About the Role

We are seeking a skilled Transition Support Worker to join our dynamic team at Rainbows Children's Hospice. As a Transition Support Worker, you will play a vital role in supporting children and young people with life-limiting conditions as they transition from children's services to adult services.

Key Responsibilities
  • Support Transition Planning: Collaborate with the Transition Lead and location-based transition teams to develop and implement transition plans for young people.
  • Provide Emotional Support: Offer emotional support and guidance to young people and their families throughout the transition process.
  • Build Partnerships: Develop and maintain relationships with external agencies, schools, and care providers to ensure seamless transition for young people.
  • Advocate for Young People: Champion the rights and needs of young people with life-limiting conditions, ensuring they receive fair access to care, education, and social activities.
Requirements
  • Experience Working with Children and Young People: Significant experience of working with children and young people with complex needs.
  • Understanding of Transition Challenges: Understanding of the challenges faced by young people transitioning into adult care.
  • Excellent Communication Skills: Ability to engage well with children, young people, and their families.
  • Motivated and Proactive: Motivated to make and sustain positive relationships with external organisations.
