Rainbows Hospice for Children and Young People is dedicated to providing comprehensive support to families throughout the East Midlands. Our mission extends beyond the confines of our facility, as we strive to assist parents and siblings during their times of grief.
Key Responsibilities
The primary focus of this role is to ensure that the ongoing requirements of young individuals with life-limiting conditions are met. We are looking for a part-time Youth Transition Facilitator to join our Transition Team, advocating for the rights of children and young adults to access essential care, education, and social opportunities. This position will require travel across various regions, necessitating the ability to navigate without dependence on public transportation. Additionally, attendance at team meetings and events at our facility is expected. The role involves flexible hours, including occasional weekends and evenings to support events such as open days and group sessions.
About Rainbows
As the sole hospice in the East Midlands dedicated to infants, children, and young adults, Rainbows provides a sanctuary for those with life-limiting conditions and their families. Our exceptional team offers end-of-life care, symptom management, and respite care, while also extending support to families in their homes and communities.
Role Responsibilities
This position offers the successful candidate the chance to work within a dynamic and supportive environment, assisting young people aged 13 to 25 as they transition from children's services to adult services. Key responsibilities include:
- Collaborating with the Transition Lead and functioning as part of a location-based transition team, serving as a resource for colleagues on transition-related matters.
- Supporting young individuals and their families through all aspects of the transition process, including health, education, and social welfare.
- Facilitating the transition of young people from Rainbows to adult services, working closely with the Family Support Team and external organizations such as educational institutions and care providers.
- Building relationships with relevant services to enhance the transition experience.

Qualifications and Experience
Essential Qualifications
- Relevant NVQ level 3 or equivalent.
Essential Experience
- Proven experience working with families.
- Demonstrated interest in the care of young individuals.
- Valid driver's license and access to a vehicle.
- Strong communication skills.
- Excellent organizational abilities.
- Effective teamwork skills.
- Experience working independently without supervision.
- Understanding of safeguarding principles for children and adults with care needs.
Desirable Experience
- Experience working with children and young people with complex care needs.
- Insight into the transition requirements of young individuals with palliative care needs.
- Ability to assess the needs of young people and their families.
- Capability to mentor and support fellow team members.
