We are seeking a skilled and compassionate Care Support Worker to join our team at Rainbows Hospice for Children and Young People. As a key member of our Transition Team, you will play a vital role in supporting young people with life-limiting conditions and their families.
Key Responsibilities- Support the Transition Lead and work collaboratively with the location-based transition team to address transition issues.
- Provide guidance and support to young people and their families in all aspects of transition, including health, education, social, and welfare.
- Work in close partnership with Rainbows Family Support Team and external agencies, such as schools, colleges, care providers, and commissioners, to ensure a seamless transition for young people moving from Rainbows to adult services.
- Significant experience of working with children and young people with complex needs.
- Understanding of the challenges faced by young people transitioning into adult care.
- Ability to engage effectively with children, young people, and their families.
- Relevant NVQ level 3 or equivalent.
- Experience of working with families.
- Evidence of an interest in the care of young people.
- Car driver with access to vehicle.
- Good organizational skills.
- Good teamwork.
- Experience of working independently without supervision.
- Understanding of safeguarding children and adults with care and support needs.
- Experience of working with children/young people with palliative care/complex needs.
- Good understanding of the transition needs of young people with palliative care needs and relevant issues.
- Ability to assess the needs of young people and families.
- Ability to teach and support other team members.
