The primary focus of our department is to enhance our medicines optimization and sustainability initiatives. The successful candidate will be instrumental in helping us meet these objectives by conducting medication histories, consulting with patients and caregivers regarding medications, planning for patient discharge, utilizing near-patient dispensing, and collaborating with primary care to ensure a seamless transition after discharge.

Main Responsibilities
The successful applicant will collaborate with specialty pharmacists and all members of the multidisciplinary team to coordinate all aspects of the patient's medication journey, ensuring effective communication for a safe and integrated transfer of care.
Key responsibilities include:
- Assisting clinical teams with the supply, storage, and management of medications in inpatient settings.
- Completing medication histories and reconciliations upon patient admission.
- Providing information to patients and caregivers about their medications, promoting adherence and understanding.
- Working closely with clinical teams to facilitate timely discharge and leave planning, ensuring medications are dispensed promptly.
- Supporting the transfer of care into primary care, utilizing the DMS service and coordinating with other healthcare professionals.
- Participating in the accuracy checking rota within the dispensary.
- Contributing to the training and development of trainees.
- Participating in weekend and bank holiday rotas.
