Position Overview

Coventry Navigation 1 (PCN) is looking to hire Pharmaceutical Care Specialists to enhance and support medication management initiatives across multiple healthcare facilities within our network.

This role presents a unique opportunity for a Pharmaceutical Care Specialist to become part of a growing healthcare network. The successful candidate will be employed by Coventry Navigation 1 and will collaborate with various healthcare facilities, receiving guidance from experienced Clinical Pharmacists within our network.

The individual in this position will engage directly with patients as part of a multidisciplinary team. The Pharmaceutical Care Specialists will oversee chronic disease management and work collaboratively with other healthcare professionals to ensure optimal patient care across the network. Key Responsibilities

The Pharmaceutical Care Specialist must be dedicated to enhancing patient health and prioritizing patient needs. Essential qualities include the ability to foster strong relationships with patients, general practitioners, nursing staff, community pharmacists, and other healthcare personnel, while effectively functioning as part of a team.

The responsibilities include:

  • Collaborating within a multidisciplinary team in a patient-centered capacity
  • Facilitating the implementation of new network services
  • Conducting comprehensive medication assessments to manage patients with complex medication regimens
  • Providing clinical medication expertise to patients and healthcare teams
  • Enhancing prescribing practices throughout the network
  • Improving service quality through clinical audits and evaluations
  • Coordinating with community and hospital colleagues to address medication-related challenges
  • Supporting initiatives aimed at improving health outcomes in care facilities alongside established healthcare teams.

It is essential for candidates to be registered with the General Pharmaceutical Council. While prior experience in primary care is advantageous, it is not a prerequisite. Flexibility, enthusiasm, and a solid foundation of clinical knowledge are crucial.

About Coventry Navigation 1 (PCN)

Coventry Navigation 1 PCN encompasses multiple healthcare facilities serving a diverse patient population. Our network operates within a dynamic healthcare environment.

The successful candidate will join a core pharmacy team and will receive support from a Senior Clinical Pharmacist and a supervising general practitioner. Regular training sessions, educational opportunities, and professional development will be provided.

Job Responsibilities

Clinical Responsibilities

• Conduct face-to-face medication assessments for patients with complex medication needs, both in healthcare facilities and in patients' residences.

• Through structured medication evaluations, assist patients in optimizing their medication use, minimizing waste, and promoting self-management.

• Provide expertise in clinical medication reviews while addressing public health and social needs of patients.

• Communicate complex and sensitive information with patients during medication assessments, including medical histories and personal circumstances, and refer to other services as necessary.

• Enhance patient and caregiver understanding, confidence, and adherence to prescribed medications.

• Lead initiatives focused on patient-centered medication optimization and quality improvement, contributing to overall healthcare quality frameworks.

• Offer general practitioners actionable recommendations and follow-up plans post-medication assessments to optimize patient treatment.

• Document medication assessment plans, recommendations, and outcomes in accordance with clinical protocols.

• Reconcile medications following patient discharges from healthcare facilities, identifying and addressing discrepancies, and collaborating with general practitioners to ensure continuity of care.

• Support the management of complex repeat medication requests from patients.

• Collaborate with hospital clinicians to proactively manage patients at high risk of medication-related complications prior to discharge.

• Liaise with general practitioners and nursing teams regarding prescribing and medication management, particularly for chronic conditions.

• Develop as an independent prescriber to manage and prescribe for patients within specific chronic disease areas.

• With appropriate training, monitor medication effects through clinical assessments.

• Participate in regular clinical supervision sessions to foster a culture of continuous professional development.

Supportive Clinical Responsibilities

• Attend relevant multidisciplinary team meetings, providing insights and feedback.

• Report medication-related incidents and contribute to investigations and reviews.

• Provide training and mentorship to other healthcare professionals on medication-related topics.

• Attend professional development sessions and participate in team-building activities.

• Ensure that all staff are informed about changes in national and local medication guidelines.

• Critically evaluate clinical evidence and present findings to prescribers during educational events.

• Lead on medication management governance to comply with healthcare standards.

• Review and support the implementation of repeat prescribing protocols.

• Conduct prescribing audits and oversee audit activities of others.

• Foster collaborative relationships between healthcare facilities and local community pharmacists.

• Take responsibility for delivering local prescribing quality initiatives in collaboration with healthcare management.

• Develop effective working relationships with general practitioners, practice staff, and other healthcare professionals.
