Clinical Support Worker

An exciting opportunity has arisen for a dedicated and compassionate individual to join our Community Learning Disability Team as a Clinical Support Worker. Under the guidance of a qualified practitioner, you will play a vital role in implementing care plans and providing support for service users and their caregivers within different community settings.
Key Responsibilities:
- Implement care plans and provide support for service users and their caregivers
- Report on the service users' wellbeing
- Participate in continued professional development, including training around Positive Behaviour Support (PBS) and Trauma informed care
- Work as part of the Community Team, undertaking short-term, focused work to maintain the wellbeing of service users
- Provide direct care, advice, and training to families and care providers to ensure support plans are in place
- Work with other health agencies to support service users in their environments under the direction of the nursing team
Requirements:
- Appropriate Learning Disability / mental health related NVQ level 2
- NVQ level 3 in LD / Mental health
- Understanding of positive behavioural support and the importance of consistency in the delivery of support and care
- Record keeping and report writing skills, including IT skills to manage electronic patient notes
- Ability to demonstrate how to Implement clinical care/care plans
- Ability to undertake observations and record these accurately
- Good computer / IT skills
Desirable:
- Special interests related to one or more of the teams priority areas (challenging behaviour, autism, active support, communication)
- Experience of working with people with learning disabilities in a range of settings
- Experience working with people who have challenging behaviour
- Experience of supporting people with a learning disability in planning and implementing activity programmes
- Working with a community team or other MDT
- Experience of demonstrating modelling activities to less experienced support workers/carers
