Job Title: Clinical Support Worker - Community Learning Disability Team

An exciting opportunity has arisen for a dedicated and passionate individual to join our Community Learning Disability Team as a Clinical Support Worker. Under the guidance of a qualified practitioner, you will implement care plans and provide support for service users and their caregivers within different community settings.

Key Responsibilities:

  • Implement care plans and provide support for service users and their caregivers
  • Report on the service users' wellbeing
  • Develop skills in Positive Behaviour Support (PBS) and Trauma informed care
  • Work as part of the Community Team to undertake short-term, focused work with the aim of maintaining the wellbeing of service users
  • Provide direct care, advice, and training to families and care providers
  • Work with other health agencies to support service users in their environments

Requirements:

  • Appropriate Learning Disability / mental health related NVQ level 2
  • NVQ level 3 in LD / Mental health
  • Understanding of positive behavioural support and the importance of consistency in the delivery of support and care
  • Record keeping and report writing skills, including IT skills to manage electronic patient notes
  • Ability to demonstrate how to Implement clinical care/care plans
  • Good computer / IT skills

Desirable:

  • Special interests related to one or more of the teams priority areas (challenging behaviour, autism, active support, communication)
  • Experience of working with people with learning disabilities in a range of settings
  • Experience working with people who have challenging behaviour
  • Experience of supporting people with a learning disability in planning and implementing activity programmes
