Position Overview:

This organization is in search of a dedicated and skilled Social Practitioner to become a vital member of their Adult Care Review Team.

As an Adult Care Social Practitioner, your primary responsibility will be to facilitate the provision of health and social care services for adults and their caregivers.

Your role will be crucial in advancing the Personalisation and Inclusion Pathways for Adults and Carers, ensuring that the legal framework of the Care Act 2014, along with strength-based and asset-based practices, personalisation, and social inclusion, is effectively integrated into service delivery.

Key Responsibilities:

  • Collaborate within a cohesive and supportive team, engaging in planning, monitoring, and evaluating initiatives alongside the Multi-Disciplinary Team (MDT).
  • Conduct needs-led assessments, manage complex cases, and develop care and support plans, while initiating multi-disciplinary assessments when necessary.
  • Provide social work assistance to individual adults, their caregivers, and families, ensuring their involvement in all planning and actions taken, while also facilitating referrals and promoting social resilience.
  • Submit funding requests and represent the organization to local commissioners or regional panels in partnership with health professionals and other stakeholders.
  • Carry out investigations involving adults at risk of abuse or neglect, adhering to the Care Act and interagency Adult Safeguarding Policies and Procedures, ensuring a prompt and personalized safeguarding response to enhance outcomes for those involved.
Qualifications:

  • Social Work qualification.
  • Registered with Social Work England.
  • Demonstrated practice that aligns with core social work values and principles.
  • Proven experience working with vulnerable adults and their caregivers.
  • Experience in conducting safeguarding investigations.
  • Experience in developing support plans and care packages.
  • Experience working with adults with complex needs.
