We are implementing the 3 Conversations model across all Adult Social Care teams, focusing on identifying individual strengths and aspirations to promote independence and well-being. We have various roles available for qualified Social Workers and Experienced Social Workers in the following areas:
Integrated Community Coordination Teams: In this role, you will conduct strengths-based Care Act assessments and develop person-centered support plans for diverse service user groups aged 18 and above. Your work will aim to enhance the well-being of adults with care needs and foster connections within their communities.
Multi Agency Safeguarding Hub: As part of the MASH team, you will support vulnerable adults facing abuse or neglect. Your responsibilities will include screening and conducting Safeguarding S42 inquiries in collaboration with individuals and partner agencies.
Deprivation of Liberty Safeguards (DoLS): We are looking for Experienced Social Workers to join our team of Best Interest Assessors. You will perform DoLS assessments for individuals who may be deprived of their liberty and provide guidance to other assessors within Adult Social Care.
Community Integrated Care Coordination (CICC): This rehabilitation service collaborates with various agencies to ensure safe patient discharges from Arrowe Park Hospital, focusing on meeting care needs effectively while respecting the individual's expertise in their own life.
Key Requirements:
- Registration with Social Work England
- Relevant social work qualification
- Evidence of Continuous Professional Development
- Valid driving license and access to a vehicle for work purposes
Successful candidates will undergo an interview process, reference checks, DBS clearance, and an Occupational Health assessment.
