Position Overview

The East Lancashire Alliance is seeking to appoint a Medication Management Specialist to support the Ribblesdale Primary Care Network (PCN).

Key Responsibilities

Medication Management Specialists play a vital role in enhancing the work of clinical pharmacists, community pharmacists, and other members of the multi-disciplinary team within the PCN. Unlike clinical pharmacists, they do not have the authority to prescribe or make clinical decisions. Instead, they operate under supervision to ensure the effective and efficient utilization of medications.

The primary objective of this role is to spearhead initiatives that optimize safe and cost-effective practices in prescribing, ultimately enhancing the quality of patient care. The individual in this position will assist patients in maximizing the benefits of their medications by transitioning them to approved protocols, refining repeat prescribing processes in General Practice, promoting repeat dispensing and online ordering, minimizing clinical risks, and striving to reduce medication waste.

About the Organization

The East Lancashire Alliance comprises a network of nine PCNs, encompassing 48 GP practices that serve a population exceeding 390,000 patients across East Lancashire. The Alliance proudly represents our member practices and advocates for our Primary Care Partners by collaborating with local general practices and other system partners to deliver patient-centered, community-based healthcare services. Job Duties

Clinical Responsibilities

- Engage in patient-facing and supportive roles to ensure effective medication use through collaborative decision-making discussions with patients.

- Execute medication optimization tasks, including effective administration of medications (e.g., checking inhaler techniques), supporting medication reviews, and conducting medicines reconciliation.

- Utilize consultation skills to partner with patients, ensuring they utilize their medications effectively.

- As directed by the PCN, assist with medication reviews and medicines reconciliation for new care home patients and synchronize medications for patient transfers between care settings, collaborating with local community pharmacies.

- Support the Clinical Pharmacist in Structured Medication Reviews (SMR) by organizing necessary monitoring tests prior to SMR.

- Provide expertise to address public health and social care needs of patients, including lifestyle advice, service information, and assistance in addressing local health disparities.

- Manage shared care protocols and collaborate with Clinical Pharmacists for more complex patient cases.

- Promote initiatives for antimicrobial stewardship to mitigate inappropriate antibiotic prescribing locally.

Technical and Administrative Responsibilities

- Assist the PCN multi-disciplinary team in ensuring efficient medication optimization processes are adhered to.

- Implement effective ordering and return processes to minimize medication wastage.

- Provide training and support on the legal, safe, and secure handling of medications, including the implementation of the Electronic Prescription Service (EPS).

- Advocate for Electronic Repeat Dispensing (eRD) and online ordering.

- Foster relationships with other pharmacy professionals and members of the multi-disciplinary team to support integration across health and social care, including primary care, community pharmacy, secondary care, and mental health.

- Assist practice reception teams in managing general prescription requests, allowing GPs and clinical pharmacists to focus on more clinically complex requests.

- Support the implementation of national prescribing policies and guidelines within GP practices, care homes, and other primary care settings.

- Aid the PCN in achieving objectives related to Quality, Innovation, Productivity, and Prevention (QIPP), Quality and Outcomes Framework (QOF), and locally commissioned enhanced services.

- Contribute to quality improvement initiatives and assist in the development of practice policies to meet Care Quality Commission (CQC) requirements.

Qualifications

Essential Qualifications

  • Professional registration with the General Pharmaceutical Council (GPhC).
  • Fulfillment of specific qualification and training requirements as outlined by GPhC criteria for registration as a Medication Management Specialist.
  • Enrollment in, or completion of, an approved training pathway, such as the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Evidence of ongoing professional development (CPD).
  • Experience working as a qualified, registered pharmacy technician in primary care, community, or hospital pharmacy settings.

  • Understanding of current healthcare policy objectives within the PCN.
