Position Overview

We are seeking a dedicated and skilled Advanced Colorectal Care Nurse to join our team at St George's Healthcare NHS Foundation Trust. This role is pivotal in providing expert nursing services within the colorectal specialty.

Key Responsibilities

  • Operate as an independent, advanced practitioner, delivering specialized nursing care in the colorectal field.
  • Oversee the development and implementation of patient care pathways, ensuring adherence to evidence-based clinical protocols.
  • Collaborate with the colorectal clinical nurse specialist team to support both surgical and oncology patients.
  • Manage a diverse caseload of patients, ensuring comprehensive clinical and social support for patients and their families.
  • Contribute to the advancement of clinical nursing practices, aligning with the latest research and guidelines.
  • Facilitate effective communication within the multidisciplinary team to promote cohesive patient management.
  • Engage with healthcare teams to enhance community care for colorectal patients.
  • Participate actively in weekly multidisciplinary team meetings to discuss patient cases and follow-up plans.
  • Deliver tailored electronic Holistic Needs Assessments (eHNA) for each patient.

Main Duties

  • Establish nurse-led telephone clinics in collaboration with consultants and outpatient departments.
  • Support the Trust in meeting compliance standards for urgent cancer referrals and treatment timelines.
  • Assist in the Lower GI surgical care pathway in conjunction with the Nurse Specialists Team.
  • Engage in Peer Review processes, ensuring compliance with mandatory measures through audits and research evidence.
  • Request and interpret diagnostic results, determining the appropriate next steps for patient care.
  • Conduct Holistic Needs Assessments and provide end-of-treatment summaries for all colorectal cancer patients.
  • Demonstrate advanced knowledge and experience in the diagnostic pathway for suspected cancers.

Qualifications

  • Registered Nurse (NMC Level 1 or Level 15) with advanced practitioner qualifications or extensive clinical experience.
  • Degree (BSc) or equivalent in a health-related field, or currently pursuing.
  • Strong leadership and autonomous practice skills.
  • Up-to-date knowledge of local and national guidelines relevant to the specialty.
  • Excellent communication skills, both verbal and written.
  • Proven ability to build rapport with patients and multidisciplinary teams.
  • Experience in the specialty area with a patient-centered approach.
  • Understanding of clinical governance and risk assessment procedures.
  • Basic counseling skills or advanced clinical communication training.

  • Experience in delivering presentations and project management.
  • Background in research and audit program management.
  • Demonstrated experience in service development.
  • Advanced clinical assessment skills or a willingness to develop these.
