Position Overview

We are seeking a highly skilled and autonomous Advanced Colorectal Care Nurse to provide expert nursing services within the specialized field of colorectal cancer. This role involves a commitment to delivering exceptional patient care and developing comprehensive patient pathways.

Key Responsibilities

  • Function as an independent practitioner, ensuring the delivery of an expert nursing service tailored to the needs of colorectal cancer patients.
  • Oversee the development and implementation of patient pathways, ensuring adherence to evidence-based clinical diagnostic protocols.
  • Support the colorectal clinical nurse specialist (CNS) team in both surgical and oncology settings.
  • Manage a diverse caseload of patients, providing clinical and social support to ensure they and their families are well-informed throughout their care journey.
  • Contribute to the advancement of clinical nursing practices in alignment with the latest research and guidelines.
  • Establish effective communication channels within the multidisciplinary team to facilitate cohesive patient management.
  • Collaborate with healthcare teams to enhance knowledge and improve community care for colorectal cancer patients.
  • Participate actively in the weekly multidisciplinary team meetings to discuss patient cases and follow-up strategies.
  • Deliver tailored electronic Holistic Needs Assessments (eHNA) for each patient.

Main Duties

  • Develop and lead nurse-led telephone clinics in partnership with consultants and outpatient departments.
  • Assist the Trust in meeting compliance targets related to urgent suspected cancer referrals and treatment waiting times.
  • Support the Lower GI surgical care pathway in collaboration with the Nurse Specialists Team.
  • Engage in Peer Review processes, ensuring that mandatory measures are met through audits and research evidence.
  • Request and interpret investigatory results to determine the next steps in patient care, including referrals as necessary.
  • Conduct Holistic Needs Assessments and end-of-treatment summaries for all patients diagnosed with colorectal cancer.
  • Demonstrate advanced knowledge and expertise in the diagnostic pathway for suspected colorectal and upper gastrointestinal cancers.
