Job Summary

An exciting opportunity has arisen for an experienced nurse to join our Specialist Community Palliative Care CNS team as a Triage Nurse.

We are seeking a highly skilled and motivated nurse to work as part of our CNS team, providing clinical triage and assessment of patients referred to our service.

The successful candidate will have excellent clinical assessment skills, be able to make clinical decisions, and have strong prioritization skills.

Main Responsibilities
  • Act as the first point of clinical contact for patients referred to our service.
  • Undertake daily effective and comprehensive triage of all new referrals to the Community Palliative Care CNS team.
  • Carry out holistic initial assessments via telephone, safety netting, and providing initial support and treatment.
  • Support with daily Palliative Care Virtual Wards and offer support to the CNS team in office-based patient administration.
  • Triage messages from patients on the current caseload to offer clinical support, advice, and assessment.

Person Specification
  • Registered Nurse with significant previous experience.
  • Post-registration experience with demonstrable experience within palliative and end-of-life care.
  • Ability to confidently prioritize workloads.
